Zoom in™ by Humana is built to pinpoint and address critical social determinants of health to reduce chronic disease risk and improve positive health outcomes.

The platform empowers Humana providers, analysts, and their community health partners to more effectively prioritize intervention resources in the community and address risk factors on a patient-by-patient basis.

The zoom in™ framework is built to ingest a wide range of geospatial data sets and convert them to a hyper-local scale. The latest release includes data for 90 social determinants of health—evaluating factors like the distance to the nearest biking path to the cost of healthy food.

With the ability to easily combine up to 7 different indicators simultaneously, a user can construct over 2 billion possible risk profiles for a given community.
